Appointments
If you would like to make an appointment, please contact our office by phone or email. If you would like to request an appointment online, please select the ‘Appointment Request’ option from the menu above. A member of our staff will follow up with you directly.
Insurance
We except all PPO's, Medicare, Tricare. We also do except Most HMO's (Prime Care, Exclusive care, IEHP-Lasell, Phoniex, Inland Faculty, Inland valley, Alpha care, Vantage, Molina. Please call our office for further questions and to verify whether your specific insurance plan is accepted.
Financial
Our practice accepts Cash, Credit and Debit Cards from all major credit card companies, Apple Pay®, Google Pay®, and Samsung Pay®. Payment plans and schedules can be discussed upon request.
Cancellation
If you are unable to keep an appointment, we kindly ask that you provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient.
